You will need to give the names, phone numbers, addresses, patient ID and date of treatment, the name of medicines you are taking, the names and dates of medical tests, and detailed information about your medical injuries, illnesses, or conditions. 

You are also required to provide information about your work. If needed, we will help supply information on:

  • How much you earned during the previous year 
  • The name and address of your previous employers 
  • Details of your active U.S. military service 
  • Information on other benefits like black lung, Federal Employees’ compensation, and workers’ compensation insurance

We will also file medical evidence like medical records, test results, doctors’ reports and award letters, settlement agreements, pay stubs, and other proof of permanent or temporary benefits.
